Product Return Instructions

Daily Balance has established a PREPAID

returns program to help you easily return a product to us at the

lowest cost. When you use this convenient return method, we will

deduct $6.00 from your product-return credit in order to cover shipping

charges. You may also make your own shipping arrangements at your

own expense and return the product to:

To return your product via the PREPAID

method, please follow these easy steps:

1. Complete the return information on the back

   of the packing slip included with your original order. Please

   make any additional notes or comments in the space provided there.

2. Package the product you are returning in its

   original packaging or the smallest box possible. Ensure the product

   is adequately padded and protected for shipping. Please be sure

   to include the Product Returns Form in the package.

3. Call FedEx at 1-866-738-8765

   or visit www.fedex.com/locatorreturns

   to find the nearest FedEx participating return drop-off location

   near you. Returns should be dropped off ONLY at the locations

   provided on the toll-free number or Web site. We suggest that

   you first contact the location to verify the days and hours of

   operation. Please note that the BedCozy and Lounge Chair

   products cannot be returned using this program.

4. Take the packaged return to one of the specified

   participating FedEx return drop-off locations. Give the clerk

   at the counter your FedEx Consolidated Returns form and the box(es).

   It is critical that you give the box to a person at the counter.

   Do not drop the package in the FedEx Express Drop Box.

   The clerk will provide you with a receipt, which includes a return

   tracking number. You can track your return by this number at www.fedex.com.
